Regarding "Notification of Purpose of Use" or "Disclosure," a handling fee of 8 USD will be charged for each application. Please enclose a 8 USD postal money order with the above documents.

If the above-mentioned handling fee is not enclosed, we will contact you to inform you of this. However, if payment is not received within the specified period, it will be deemed that no request for disclosure has been made. Please note that the documents sent to us will not be returned in principle.

Choose one of the following:

  • Driver's license, driving record certificate, passport, personal identification number card (only the side with name and photo), disability certificate, mental health certificate, developmental disability certificate, residence card, special permanent resident certificate.

Choose two or more of the following documents:

  • Proof of public health insurance coverage, copy of resident registration, resident registration record certificate, pension book, insured person's certificate for later-stage elderly medical care or nursing care insurance, union member card for the national or local government employee mutual aid association, subscriber certificate for the private school teachers' mutual aid system, child-rearing allowance certificate, special child-rearing allowance certificate.

The personal identification documents you provide will be used as follows. Please send them only if you agree to the following conditions:

  • Your personal information will be used to respond to requests for disclosure made by you.
  • Personal information that is not necessary for identity verification should be redacted or obscured to prevent it from being read.
  • Except as required by law, we will not provide your personal information to third parties without your consent. We also do not plan to outsource the handling of your personal information.
